Aircraft Accident investigation is not about how, but about why
Everything I write About I believe challenges decades of poorly investigated aviation accidents, which resulted in incorrect probable causes, poorly written findings and ineffective recommendations.
I call into question what was accepted, what we were told. I believe in finding the true root causes of aircraft accidents in the hopes of preventing similar accidents in the future.
I accomplish this by analyzing past accidents to learn why the probable causes never captured the truth; why the findings were unrelated and why the recommendations led nowhere. I do this by giving a fresh perspective to what we know was never right. then we can unlearn what was written in error.
